The Practice

1. We will support the emergence of an improved Cluster model for Wales where groups of practices have the funding and autonomy to do the work that matters, delivering coherent and equitable offers to communities across Wales, valued by stakeholders across the system.
2. Improving the resources available to support practice resilience and sustainability.
3. Widening access and education projects will contribute to the workforce of the future.
4. Creating the conditions where new roles can be successfully embedded in practice will diversify practice workforce and enable inter-professional learning in primary care.
