Yma secured Level 1 SMART FIS funding from the Welsh Government to pilot integrated community wellness hubs in partnership with the Football Association of Wales and Cardiff South West Primary Care Cluster.

GPs delivered lifestyle medicine interventions for people who were recently diagnosed with Type II diabetes in Ely Rangers Football Club. The interventions were deisgned to enable effective ongoing self-management and improvement in health, which was observed over the 10 weeks project period.

Yma is now developing an idea for a larger scale project in marghinalised communities.
